Vet Referral

Vet Referral Form I work within the guidelines of the Veterinary Surgeons Act 1966, which states that only a Veterinary Surgeon may make a diagnosis and treat an animal. Veterinary referral must be obtained before physiotherapy treatment can be performed on any animal. 

 

All clients must have received veterinary consent prior to treatment.
